You will need a bug out bag to put your key survival items into. This should be prepared ahead of time to ensure that you are ready at a moments notice. Being prepared ahead of time is key to any survival situation. Assume that you will need a minimum of 3 days worth of consumable items. If you think you will need to be gone longer prepare accordingly. The ideal survival kit will include most of the items below but you will need to make the ultimate decision as to what you will need.

1) Your medications for at least 3-days and more if you plan on going longer. If you plan on going out for a week you should plan to take extra in case you are delayed.
2) Compass and map of your area, preferably topographic. You might want to include a GPS system as well but remember these can fail because of lack of batteries or breakage. A good compass will always work if properly packed.
3) Paper and pencil. Useful for leaving messages for would be rescuers, drawing maps for later reference, etc.
4) Fire making materials. Choose more than one, three is best: lighter, matches, magnesium fire starter, or firesteel.
5) Tinder. You can use cotton balls soaked in Vaseline or purchase a commercial fire starting product.
6) Candle. Try to obtain a long burning 8-hour type candle. Can be used to help dry out fire starting materials, as a source of light, or heat.
7) A large sheet of aluminum foil. Can be used to form a pot to boil water in or cook some food.
8) A multi-tool, something with screwdrivers, pliers, toothpick, scissors etc.
9) Fixed blade survival knife. A SOG Seal Pup or a Becker BK7 are excellent choices. Try to find a full tang knife with a relatively heavy blade which can be used for cutting and splitting wood.
10) At least 2 quarts (2 liters) of water. This is an amount that is good for a day.
11) A way to purify water. Boiling is by far the best method but you may choose a filter or water purification tablets such as chlorine or iodine. You can also buy a Katadyn water filter system to be able to purify available water. This way you won't have to carry a large quantity of water with you and you won't need to stop to boil water.
12) A metal cup or other cookware. This will enable you to boil water or cook some food.
13) Rain gear. Extra clothing. In order to stay warm you must remain dry.
14) Tarp or space blanket. This will enable you to create a shelter for the night, possibly construct a debris shelter (use of dried leaves or vegetation to help insulate you from the environment and the ground).
15) 25 to 50 feet of cord, preferably 550 paracord. This is very strong 550 lb tensile strength. The cord can be taken apart and the internal threads can be attached together to form a fishing line, or used to help set up your tarp as a shelter. This has many uses.
16) Flashlight or LED headlamp. You never know when you will be caught out in the dark and need to find your way back. Make sure you remember extra batteries.
17) Time piece or a watch.
18) Signal mirror. If you are lost and need to attract attention you can signal them with a mirror or your headlamp at night.
19) Survival whistle. It is a lot easier to blow a whistle continuously than it is to yell. It can also be heard a lot further when you are signaling for help.
20) Large garbage bag or even better a 50-gallon drum liner. Useful for a shelter or to help keep you dry.
21) A good first aid kit. Insect repellent, sun glasses, and sun screen. Remember you will not want to be out in the sun during the heat of the day if you have limited water. Stay in the shade to conserve your energy.
22) Survival food backup. This may include trail mix, protein bars, nuts, various candy, etc. I like to make jerky to keep in my bug out bag. Jerky is primarily protein, can last a while without refrigeration and is very lightweight. Dehydrated foods are ideal for a bug out bag because of their lightweight and reduced size. Depending upon the season you can find food on the trail or do a little fishing if you have included a couple of hooks and some sinkers in your bag.
